That is what is left of the broadcast sheet for your car. It has the codes that designate what type of trim, color, options and such that your car will have as it makes its way down the assembly line. It is usually tucked between the springs of the back seat.
 
Not the original seat(s) for your car, maybe? Lot's of '69 and later codes on there. Also a boatload of codes I don't have reference for - hopefully Doug can shed some light. Not even sure "what" this car is....full size Plymouth 4-door; Canadian built??? Green inside and out? - first time I've seen this format. Neato!

I lucked up on this car she’s a little rough but I’m trying to straighten it out
 
I find this sheet intriguing with its jumbled code-mix, and so many of which I can find no reference for. I'd consider it "something other than" a broadcast sheet as we are accustom to. I made a pass-through on it.
@69CoronetRT - can you add any insight on this? A May SPD is somewhat late, but not really. It almost seems like the computer system was 'midway' on the changeover from "68 and prior" coding to "69 and later" coding? Is this all just peculiar to Canada? - I haven't been able to find Canadian ordering/coding books/forms as-of yet but continue to look.

Nice work! I think you are correct on a lot of what you decoded

Could be the time the sheet was printed or the sequence number of sheet printed.

Due to placement next to interior code, this likely indicates the inner upper door frame paint Green to go with U/G on the tag.

The Windsor plant coded differently than US. I would not try and make letter & two digit codes 'fit' with 69 and later codes.

Typically a broadcast sheet will also show assembly numbers for major components. For example, the last code, 690, is the engine assembly code for a 68 C body 318 with A/C. Broadcast sheets from US built cars would code other assemblies or parts. This could be what is indicated by the lettyer/number codes on a Windsor plant sheet.
